Full-service Fee Schedule

In accordance with the Service Agreement between the Employer and LT Trust Company, Inc. (LT Trust) (the “Agreement”), the Services listed below (and further described in the Agreement and List of Services) will be provided by LT Trust for the fees indicated. Optional services will be provided for the fees stated if so requested by the Employer or its Authorized Agent in writing.
